Participant eligibility

Individuals with the following criteria may be eligible to enroll at Home Avenue ADHC
​
-
18 and over with one or more chronic or post acute medical, cognitive, or mental health conditions diagnosed by a personal healthcare provider.
​

Limitations with activities of daily living, such as bathing, dressing, self feeding, toileting.​
​

At high risk for hospitalization, institutionalization, or deterioration.
​
-
Conditions require adult day health care services such as nursing, personal care/social services, therapeutic activities, and meal service.
​

Insufficient support to safely maintain in the community, for example no family or caregiver available.

Our staff will organize the transportation for your loved one from your home to the center and vice versa. From there we provide nutritious meals, social interaction, recreational activities, and a caring environment. Our services also help family members to be temporarily relieved from caregiving duties. We strive that our center and friendly staff will bring balance for family life at home.
Admission Procedures
If you are interested in attending home avenue we invite you to schedule a tour of the facility. Our social work team will explain the admission process and discuss any other questions you may have.

Cost for CBAS Services
Community Based Adult Services are covered by Medi-Cal with a managed care plan or private payment. If the potential participant is eligible for Medi-cal the center obtains prior authorization form the enrolled Medi-Cal managed care health plan or local Medi-Cal office to provide services. If the potential participant does not have Medi-Cal they are considered private pay. Please contact the center for private pay rate.

Enhanced Care Management (ECM)
Home Avenue has extended it's delivery of services through CalAIM Enhanced Care Management and Community Supports in partnership with LA Care. Enhanced Care Management is intended for Medi-Cal individuals with complex needs who face difficult life and health circumstances. ECM "breaks down the traditional walls of health care"-- extending beyond hospitals and health care settings into the community. ECM is a benefit for populations of focus addressing clinical and non-clinical needs through intensive coordination of health and health-related services. ECM participants will be designated a lead care manager who will help coordinate care and services among the physical, behavioral, developmental, and social services delivery systems, making it easier for them to get the right care at the right time.
