Founded in 2003, Home Avenue ADHC/CBAS has worked with local agencies for over 10 years to provide essential daily care to families in Los Angeles and Orange County areas. HomeAvenue ADHC/CBAS is a unique program of its kind serving the Los Angeles County and some Orange County areas. We are licensed by the California Department of Public Health and Aging, and have a proven history of excellence in providing care to adults who are coping with chronic medical conditions.

Our friendly skilled staff of health care professionals are here to help you achieve optimum health so you can “live life to the fullest”. We seek to expand health care and human services to provide for the growing needs of the frail, elderly, and disabled adults. We serve persons with cognitive impairments such as Alzheimer’s, persons with mental health issues, persons with chronic medical impairments, and persons with disabilities. We serve the Service Area of Los Angeles and some areas of Orange County.
Who is eligible to participate from Home Avenue ADHC/CBAS Services?
Participants must be eligible for benefits and meet Medical Necessity Criteria. A Face-to-Face Evaluation may need to take place by the Participant’s assigned Medi-Cal Managed Care prior to the comprehensive interdisciplinary assessment.
Who do we serve?
Adults 18 and over with chronic medical/mental impairments who have limitations with their Activities of Daily Living or Instrumental Activities of Daily Living.
Examples include: Participants with Diabetes, Alzheimer’s, Dementia, Hypertension, Depression, Anxiety, etc.
Adult Day Health Care is covered by Medi-cal FFS (with exemption) and/or Medi-Cal Managed Care benefits.
